Survey Readiness Assessment

1. Our F-Tag binder is current and every department leader knows where it lives.

2. Mock surveys are conducted at least twice per year.

3. Our QAPI committee meets monthly with documented minutes.

4. Care plans are updated within 7 days of any significant change.

5. Infection prevention rounds happen daily with documented findings.

6. Every new hire completes a documented competency check before independent practice.

7. Kitchen and med pass observations are audited monthly.

8. Grievance logs are reviewed and trended quarterly.

9. Staff can articulate the current CMS focus areas without prompting.

10. Leadership rounds on every unit at least once per shift.
